Prior to the employees termination date, you will need to submit a request via email to your Account Manager. Please include the employee's original hire date, their termination date, and the time period (benefits start and end date) for which you would like to extend benefits. Please note, only health and dental benefits can be extended beyond Employment Standards Act guidelines and are subject to approval of the insurance carrier.
