A claim can be denied for several reasons. If your employee group benefits claims was declined when being billed directly from your pharmacy or service provider, it may be that information was entered incorrectly (name spelling, incorrect birthday), the claim is not a covered expense under your plan, or it was denied in error. A claim may also be denied if more information to support your claim is required, such as a physician's note or treatment plan.
